Description: The RN implements professional nursing practice centered on the patient as a whole person. The focus is on creating a caring environment centered on healing and wellness. Nursing practices are based on evidence, current research, and department policies and procedures. Clinical decision-making incorporates all phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among staff and across disciplines. The RN provides direct patient care activities on the unit. This includes nursing rounds, assessments, and treatments. The RN facilitates admission, transfer, and discharge activities. Provision of patient and family-centered education is also required. The RN coordinates care and develops individualized treatment plans as applicable. The RN performs and documents comprehensive assessments in accordance with scope of practice and policies. Consideration of the patient’s culture, age, and developmental level is essential. Identification of patient risk factors and presenting complaints is a key responsibility. The RN obtains health information from family or others as necessary. Both objective and subjective data are collected for a complete assessment. The RN verifies initial admission assessments if obtained by other team members. A problem list is developed, prioritizing patient needs in collaboration with the inter-professional team. Actions and interventions are designed relevant to the patient’s culture and age. Desired outcomes are determined in coordination with the patient and family, following the medical plan of care. The RN fully implements patient care therapies and procedures within scope of practice. Procedures, treatments, and processes are explained to patients and families. Actions to alleviate patient stress and discomfort are initiated promptly. Recognition and communication of changes in patient condition are crucial. Ongoing assessments and responses to procedures and therapies are documented accurately. Delegation of tasks is performed appropriately, overseeing the performance of activities related to patient care. The RN monitors the accuracy and completeness of patient care provided by team members. Educational needs of patients related to prioritized problems are identified and addressed. Facilitating the discharge process includes executing discharge orders in a timely manner. Collaboration with case management and others is necessary for a smooth discharge. Input into patient/nurse assignments is provided, considering staffing and patient acuity. Active participation in unit-based activities in support of organizational values and priorities is expected. The RN promotes a positive group dynamic and healing environment through words and actions. Assurance of nursing excellence is in accordance with accepted standards and evidence-based practice. Participation in quality improvement activities and core measures is encouraged. A holistic perspective regarding patient care is utilized throughout.
